To Request Representative Payee Services
In order to become your representative payee we need you to submit a completed application and an SA 787 completed by your physician.
To Request Power of Attorney Services
Forms for Estate POA: Intake Form, Estate Services for Bill Pay Only, Estate Services General Checklist, and the 2024 Fee Agreement. Please note on the page that all forms must be completed and returned to Thompson Social Services at 39 S. Tulpehocken St. Pine Grove, Pa 17963 or faxed to 570-915-6199.
